    In this role you will work with the RN Case Manager to identify and submit Potentially Avoidable Day reports. Participate in the coordination of acute transfers to alternative level of care as appropriate, including acute psychiatric facilities. You will be given resources to perform documented assessment and reassessment of the patient’s status through the hospitalization and in accordance with CMS and California Children’s Services regulations. You will need to collaborate with multi-disciplinary team to initiate complex discharge planning on identified high risk patients in order to ensure a timely discharge and provide appropriate linkage with post-discharge care providers. Additionally, collaborate with patient/family/physician, community agencies and other care providers to determine continuing care needs- assesses and refers to appropriate interagency resources to expedite a safe discharge plan. Collaborate with the Case Management team to safely discharge patients. Expectations are to provide crisis intervention, information and referral services to patients, families and hospital staff, according to identified needs. Your experience should demonstrates expertise and provide intervention as appropriate in the following areas: child abuse/neglect; domestic violence; guardianship temporary/permanent); foster care; adoption; mental health placement; advance directives; and adult/elderly abuse.

    Your tasks include submitting written performance activity logs of daily Social Work activities to manager. Develop and maintain a comprehensive knowledge of the community and interagency referral network to enable a smooth transition of patients to needed post-acute services. Facilitate Critical Incident Stress Management (CISM) intervention services as appropriate. Facilitate and participates in end of life/withdrawal of life support referrals that may include hospice or palliative care interventions. You’ll be allowed to document oversight and interactions with medical team regarding patient and/or family information and response to the discharge plan. Actively participates in appropriate community committees.


    Minimum requirements include; Master’s degree in Social Work (MSW). One year Case Management experience in a clinical setting. Current BLS certification. California Children’s Services (CCS) paneling within 6 months of hire or entry into job.
